4. Background Check Authorization

I understand that a satisfactory background check is required before beginning any volunteer role involving direct contact with program participants. By signing this agreement, I authorize OnRamp To Independence to conduct a background screening, which may include criminal history, sex offender registry checks, and reference checks.

I certify that all information provided in connection with this background check is true and accurate. I understand that OnRamp may decline my volunteer application based on background check results, and that I have the right to review and dispute any findings.

5. Assumption of Risk & Liability Release

I understand that volunteer service with OnRamp To Independence may involve activities that carry certain inherent risks, including in-person program activities, transportation, and interactions with vulnerable youth. I voluntarily assume those risks.

To the fullest extent permitted by Georgia law, I hereby release, waive, and discharge OnRamp To Independence, its officers, directors, staff, and agents from any and all claims, demands, and causes of action arising out of or related to my volunteer service, including personal injury, property damage, or any other loss.

I understand this release does not apply to claims arising from OnRamp's gross negligence or willful misconduct.
