Photo & Media Consent and Release

Permission to photograph, record, and use your image or likeness in OnRamp To Independence materials.

This form is voluntary. Choosing not to sign will not affect your access to OnRamp programs or services. We will never share images that identify you as a foster youth or program participant without your explicit permission.

2. Grant of Rights

By signing this form, I (or the parent/guardian of the subject named above) grant OnRamp To Independence and its authorized representatives permission to photograph, video record, audio record, and otherwise capture my (or the minor's) image, voice, and likeness in connection with OnRamp To Independence programs, events, and activities.

I grant OnRamp To Independence a non-exclusive, royalty-free, perpetual license to use, reproduce, edit, publish, and distribute this content for the purposes checked below, without further compensation or notice.

5. Acknowledgment & Privacy Statement

Privacy Protection: OnRamp To Independence will never use photography or media to identify, disclose, or imply that an individual is a current or former foster youth, program client, or recipient of services without explicit additional consent.

Waiver: I waive any right to inspect or approve the finished product in which my likeness may appear, except as specifically noted above regarding video testimonials.

Revocation: I understand I may revoke this consent at any time by contacting OnRamp To Independence in writing at info@onramp-independence.org. Revocation will not apply to content already published or distributed in reliance on this consent.
