Background Check Authorization

List all addresses from the past 7 years. Attach additional sheets if needed.

The background check conducted by OnRamp To Independence may include, but is not limited to:

• Criminal history search (federal, state, and county records)
• National sex offender registry search
• Georgia Child Abuse Registry (where applicable)
• Address history verification
• Identity verification
• Professional reference checks (if applicable to the volunteer role)

Background checks are conducted by a third-party consumer reporting agency (CRA) selected by OnRamp To Independence. Results are maintained confidentially and used solely to evaluate volunteer fitness for direct-contact roles with youth and young adults.

I hereby authorize OnRamp To Independence and its designated consumer reporting agency to conduct a background screening as described above.

I certify that all information provided in this form and in any associated application materials is true, accurate, and complete. I understand that providing false or misleading information may result in immediate disqualification or removal from a volunteer role.

I understand that:

• I have the right to request a free copy of any consumer report obtained about me.
• If OnRamp declines my volunteer application based on background check results, I will receive written notice and information about the reporting agency.
• I have the right to dispute any inaccurate information in the report directly with the consumer reporting agency within 30 days of the notice of adverse action.
• A criminal record does not automatically disqualify me. Each situation is reviewed on a case-by-case basis, taking into account the nature of the offense, the time elapsed, and the volunteer role in question.
